I wouldn't call that a comprehensive privacy policy myself, but I guess it is there. It is not linked to from the questionnaire, however. And I'm glad the server is secure, though the data is being hosted in one jurisdiction and the company analysing it is based in another, and how they plan on analysing it and anonymising it I do not know, I just know that I do not see a green padlock in my browser when I click the survey link. Maybe it's just me.

I agree with you, Dorothy, that some of this information should not be collected. I do think it is reasonable to collect basic demographic data from respondents, i.e. one's geographic region and community affiliation (though I think these fields should be drop-down menus rather than text boxes), but I am not sure why one is being asked to provide their name in order to complete the questionnaire. That said, and while it is not stated on the form, I believe the survey does allow respondents to proceed to the next page and to ultimately submit the form even if they have left the field requesting their name blank.

This is perhaps a meaningless gesture, however, given that the questionnaire is not hosted on a secure server, there is no privacy policy that I could see, and it is not to me clear how the independent assessors intend on anonymising the data (I suspect it would border on impossible to do so). And a survey like this one should really be translated and available for completion in multiple languages — as best I am aware, the Ombuds function operates primarily in English, so the satisfaction or dissatisfaction of someone with the Ombudsperson who does not speak English could really influence the results...
